Tomato Plant Dying

One of my tomato plants seems to be dying. The symptoms are yellow leaves, that then drop off. About half the leaves are already gone.

There is a pest, a green worm in my tomato garden that is chewing on the leaves of the plant.

I went out one evening to pick some tomatoes from our garden and found a huge green worm chewing on the leaves of the plant. The worm was about 3 inches long and thick bodied with two pincher like appendages. It was the same color green as the tomato leaves.
